HIPAA stands for Health Insurance Portability and Accountability Act, which is the National standards to individual's medical records protection, also involves personal health information as health status, health plans, even healthcare providers.

The fact that HIPAA are National standards means that all organisations related to it must obey those rules for information privacy and protection. So, HIPPA isn't shaped by health care providers or "state regulations", it's the opposite, all organisations related have to apply according to the mandatory standards established.
